We do require:

1. Proof of your professional license.
2. Proof of your NCC and/or AWHONN membership.
3. A definition of the nature of your perinatal education program.
4. Tax ID number (this may be a SSN in some states). If your state requires a tax ID - we must also.
5. A $250 minimum yearly purchase from date of approval. Approximate cost as low as $7.50/participant.

We feel that these programs offer a substantial value and you can tell from our requirements, we are only offering it to qualified professional nurses in the field of obstetrics.
